The Department of State will say this application as promptly as possible after its receipt Inquiry concerning it should be made only in case of emergency, since the time consumed in responding to such an inquiry may retard the issue of a passport to you and would necessarily retard the issue of passports to other persons.

An application for a passport must have attached to it a copy of the applicant's photograph.

A loose signed photograph of the applicant must accompany the application.

The photographs must be on thin paper, should have a light background, and be not over three inches in size.

The legal fee for a passport is nine dellars, in currency or postal money order, and one delier for execution of application.

The total fee is $10.00. It is unnecessary to pay any other fee to any person in connection with the filling out or execution of this application or for obtaining the passport. The clerk of court or Passport Agent balore whom this application is executed will give the applicant all necessary information and guidance.

A person born in the Unite States should submit a birth or baptismal certificate with his application, or if the birth was not recorded, affidavits from the attendng physician, parents, or other person having sumcient knowledge to be able to testify as to the date and place of the applicant's birth. The same kind of evidence should be submitted if American-born wife is included in the application and the marriage occurred on or after September 22, 1922. If wife acquired American citizenship through naturalization on or after date just mentioned evidence of her citizenship of the nature set forth in the Rules Governing the Granting and Issuing of Pa ports in the United States should be submitted.

passport applications since 1915. This attests to the fact that a passport is a document of protection as well as one of identity and nationality of the bearer.

Data concerning the nationality of the father of the passport applicant has been required on all applications since 1889. This information particularly applied to naturalized citizens and individuals born in the United States of foreign-born parents or born abroad of American parents and was necessary for the protection of the bearer in the event a foreign state claimed a person had dual citizenship.

In 1917, the application form underwent major revisions including the requirement to provide details of the citizenship of the father of the applicant, the applicant's residence outside the United States and information regarding previous passports issued to the applicant. (Exhibits 83 and 84) Additional minor changes may be found in the application forms printed between 1917 and 1927. (Exhibits 85, 86, and 87)

The application of 1927 contained a combination Clerk of Court/Passport Agent format. In 1937, the form was reduced in size from 84 inches by 14 inches to 8 inches by 1212 inches and expanded to four pages consisting of two parts. Part II on page 3 was used only if family members were to be included. (Exhibits 88, 89, and 90) Conditions following World War II caused a major change in format on the 1946 application form. (Exhibits 91 and 92) The following statement was added:

I solemnly swear that the statements made on both sides of this application are true and that the photograph attached hereto is a likeness of me.

I have/have not been naturalized as a citizen of a foreign state; taken an oath or made an affirmation or other formal declaration of allegiance to a foreign state; entered or served in the armed forces of a foreign state; accepted or performed the duties of any office, post or employment under the government of a foreign state or political subdivision thereof; voted in a political election in a foreign state or participated in an election or plebiscite to determine the sovereignty over foreign territory; made a formal renunciation of nationality before a diplomatic or consular officer of the United States in a foreign state; been convicted by court martial of deserting a military or naval service of the United States in time of war; been convicted by court martial or by a court of competent jurisdiction of committing any act of treason against, or of attempting by force to overthrow, or of bearing arms against, the United States.

A reorganization of the Passport Office, including files, forms, procedures, and issuance, brought about changes in the application form. The application was reduced from 8 inches by 121⁄2 inches to 8 inches by 102 inches on December 15, 1955, and made into a booklet-type format with sections of the form set in block type. The language was simplified; the forms for native-born and naturalized citizens and for accompanying dependents were consolidated into one form; questions concerning Communist affilia
