affidavit of another person who attested to his citizenship. This was modified in 1888 to allow a certificate to suffice provided the additional information was furnished:

Every applicant is required to state his occupation and the place of his permanent legal residence, and to declare that he goes abroad for temporary sojourn and intends to return to the United States for the purpose of residing and performing the duties of citizenship there.R

Forms proscribed in 1888 required separate forms of passport application for native citizens, naturalized citizens, and persons claiming citizenship through naturalization of husband or parent. Previously only one form of passport application was used. The application forms illustrated consist of three pages, measuring 81/4 inches by 14 inches, and differ slightly from those used in 1888. (Exhibits 77, 78, and 79) These forms were revised September 15, 1896.9

The 1905 application was the first issued in an affidavit format with a provision that it be sworn to before a notary public or a passport agent. (Exhibit 80) Before the passport could be granted the applicant had to state his citizenship, his identity, and, as a rule, his permanent residence in the United States. He also had to affirm his intention to return for the purpose of performing the duties of citizenship.10

The passport applicant was required to be introduced if possible by a reputable individual known to the office accepting the application, or he was required to identify himself by satisfactory documentary evidence. In questionable cases, the Department could check the U.S. references which the applicant had to furnish.

Native citizens were required to submit a birth certificate, if possible, or letters or other documents that established citizenship. Naturalized citizens were required to submit a certificate of naturalization, name of the court in which they obtained naturalization and date of naturalization or a certified copy of the court record, or a previous passport issued by the Department. The 1914 domestic application form consisted of two pages: the first page included a space to list the countries to be visited and the object of the visit and the second, a space for the photograph. The form stated that the application must be subscribed to before a clerk of court.11 (Exhibits 81 and 82)

The application must be in duplicate and accompanied by three unmounted photographs of the applicant, not larger than three by three inches in size, one of which is to be affixed to the passport by the Department; the other two must be attached to this application and its duplicate, respectively. The photographs must be on thin paper and should have a light background. The one not attached to the application should be signed by the applicant across its face, so as not to obscure the features.
